Simple fix to time issues, I guess- just mandate that everyone on the roster has to average at least 5 minutes a season, and no more than somewhere between 25 and 30.
You don't have to exclusively stick to salary cap penalties either- you could make it so that anyone who plays less than a certain number of minutes per game over a season can end their contract early and declare themselves a free agent.

Who cares how much a GM rides his top guys? The only thing I see as an issue is active players getting jealous of ice time in which case they can ask to be traded or walk as an FA which are both exciting and good for the league imo.
